This position provides a variable level of support to consumers with mental health issues in an integrated Psychosocial Rehab treatment team structure. This includes but is not limited to providing assistance in accessing medical or other appropriate support services as well as linkage to more intensive services, if needed, in addition to monitoring (monthly or weekly as needed), assessment of service needs, administration of TIMA medication management functions. This position requires intervention and frequent in-person, long-term contact with individuals, families, and service providers. This position requires the ability to: identify problem areas; work under pressure and stressful situations in a professional manner; be thorough in assessment and documentation; locate and utilize a variety of community resources to meet the needs of each individual; assist in negotiating and advocating for each individual with service entities outside of the Center; and develop a positive relationship with each consumer. This position reports to the MH Program Director. This position is responsible for coordinating services with other Center staff, outside agencies, and civil authorities to provide services for individuals on the LOC3 and ACT Team. This position is responsible for assisting in maintaining an up-to-date community resource network. Services are provided in the office, community, and at other sites in the Center catchment area (Taylor, Jones, Callahan, Stephens, and Shackelford Counties). Services and documentation of those services will follow all state and federal standards, including Medicaid requirements. This position must consistently maintain monthly performance targets.
QUALIFICATIONS:
• Registered Nurse License. Psychiatric certification preferred.
• Experience in mental health/intellectual and developmental disabilities preferred.
• Must have a valid Texas Driver's License and be an insurable driver.
